Systematic review and meta-analysis of 82 RCTs (10,332 patients) comparing probiotics vs placebo in IBS. For global symptoms: moderate certainty only for Escherichia strains (L. reuteri DSM 17938), low certainty for Lactobacillus plantarum 299V. Very low certainty for most other combinations. Certainty of evidence by GRADE was low to very low across almost all analyses. Some combinations may be beneficial but firm recommendations cannot be made.
